Description:

Resources Extended to Assist the Chronically Homeless (REACH) is a HUD funded street outreach and case management program for individuals and families who stay in camps, vehicles, and under bridges. The purpose is to bring people into shelters and/or housing, while addressing employability and substance abuse concerns.

Hours:

Monday through Friday 8 a.m. to 4:45 p.m.

Intake Process:

Materials required for intake include (if available):
- proof of homelessness,
- birth certificate,
- social security card, and
- proof of income.

Program Fees:

None

Languages:

Translation services are available.

Eligibility:

- Program primarily serves individuals who are literally homeless (living in camps, vehicles or other places not meant for human habitation) who need assistance with finding shelter/housing.
- Limited Financial Assistance Available
