Description:

All CHAMP case management referrals must go through the Homeward Bound Office and will be assigned to the appropriate case manager. Primary Referral "Case/Care Management."

The CAC Case Management Project is a program that serves at-risk residents at the following KCDC High Rises: Love Towers, Northgate Terrace, Isabella Towers, Cagle Terrace. Our goal is to keep residents in housing and to prevent evictions to the streets.

Hours:

M-F: 8:00AM - 4:45PM

Intake Process:

A referral from KCDC High Rise managers is required.

Program Fees:

No program fees.

Languages:

Translator services are available.

Eligibility:

Residents must be identified by apartment manager as "at-risk" of losing housing, or under eviction.

Apartment managers send referrals to case managers who will provide case management from that point.
