Description:

The CAC Homeward Bound County Outreach Program serves the growing number of individuals who are living on the streets, camps, cars or other places not meant for human habitation in Knox County outside the City limits. One Outreach Case Manager provides outreach to assess each client!€™s individual needs and provides referrals and linkage for the programs and services for which they qualify. Re-housing assistance is also available through the program.

Hours:

Monday through Friday 8 a.m. to 4:45 p.m.

Intake Process:

Materials required for intake include (if available):
- proof of homelessness,
- birth certificate,
- social security card, and
- proof of income.

None

Languages:

Translation services are available.

Eligibility:

- Program primarily serves individuals who are literally homeless (living in camps, vehicles or other places not meant for human habitation) who need assistance with finding shelter/housing.
- Limited Financial Assistance Available
