Description:

All CHAMP case management referrals must go through the Homeward Bound Office and will be assigned to the appropriate case manager. Primary Referral "Case/Care Management."

This department provides a variety of programs to assist the homeless achieve self-sufficiency. All programs offer comprehensive and integrated case management, outreach, crisis assistance, job, life, and basic skills training, GED preparedness, budget management, parenting classes, employability classes, housing placement and transportation assistance and other supportive services related to a customer�s goal of achieving self-sufficiency.

EMERGENCY SOLUTIONS GRANT PROGRAM (ESG) - Links homeless people with resources to housing, employment, or job training.

FAMILIES IN NEED (FIN) - Provides homeless families with case management, comprehensive family development planning, supportive services, and life skills training to prepare for, and maintain permanent housing.

PROJECT SUCCEED - Provides job training, case management, life skills, and GED preparation to homeless individuals who reside in shelters, transitional housing, and halfway houses. Parenting, employability, and budget classes are taught on a weekly basis.

REACH (Resources Extended to Assist the Chronically Homeless) - A street outreach program providing case management, support services, life skill training, and housing assistance to the street homeless.

Eviction or Detainer Notice (for Homeless Prevention Assistance)
Proof of Household Income
If Domestic Violence Victim- order of protection or letter from agency that can verify status preferred but self declaration accepted

Eligibility:

For Homeless Assistance: Household must meet HUD's Literally Homeless Definition and not have any other social or financial supports available to help regain housing. For Homeless Prevention Assistance: Assistance available to households with families with children whose household income must be at or below 30% of Area Median Income who are at risk of losing their housing in the next 21 days and no appropriate subsequent housing options have been identified and no alternative social or financial supports for assistance are available.
